اهمية الفلور فى منع او الحد من انتشار مرض تسوس الاسنان‎

الفلور هو عنصر كيميائى مشتق من اللاتينية (fluere) والتى تعنى السريان، عدده الذرى 9. ويكون فى الحاله الغازية فى درجة الحرارة العادية وله مواصفات أخرى. ومايهمنا هو علاقته بالاسنان

يدخل الفلور فى تركيب العظام بشكل عام، و تركيب الأسنان بشكل خاص، وبالذات فى طبقة المينا التى تمثل الجزء الخارجى من السن.

يؤدي نقص عنصر الفلور بنسب معينة فى مجتمعات مختلفة إلى مرض تسوس الاسنان، كما يؤدى الى حالات التواء فى العمود الفقرى مما يسبب تأخراً فى التئام العظام المكسوره.

من ناحية أخرى، تؤدي الزياده فى نسبة وجود الفلور إلى ظهور بقع غامقة اللون على الأسنان تؤثر نفسيا على حالة الشخص المصاب. من الممكن علاجها إما بعملية تبييض الاسنان، واما تلبيس او تغليف الاسنان باللون الذ ى يرضى ذوق الشخص المصاب و ذلك حسب الحاله، و يتم هذا فى عيادة طبيب الاسنان بعدة طرق مختلفة.

مؤشرات

- يقل تسوس الاسنان فى المجتمعات التى يحتوى فيها الماء على النسبة الصحيحة من الفلور

- تقل نسبة الاصابة بتسوس الاسنان لدى الاشخاص الذين يستخدمون معاجين اسنان ومضمضات تحتوى على نسبة من الفلور

- تقل نسبة الاصابة بتسوس الاسنان لدى الاشخاص الذين يستخدمون الملح الذى يحتوى على نسبة من الفلور وكذلك الحليب

- يؤخذ الفلور عن طريق الدم اثناء فترة تكوين العظام وفترة نمو الاسنان

- عند البلوغ تقوم الخلايا فى الجسم بفرز الفلور بشكل سائل

- بعد الانتهاء من عملية بزوغ الاسنان لدى الاطفال تقوم الغدد اللعابيه بافراز اللعاب الذى يحتوى على الفلور باتجاه الاسنان

- يتم تحويل بلورات الهيدروكسى ابيتات الى بلورات الفلورو ابتايت عندما يتم تطعيم الاسنان بالفلور

- ان حجم البلورات يصبح اكبر مما يساعد على تاسيس اسنان مقاومة لمرض تسوس الاسنان

- بعد بزوغ الاسنان يكون عمل الفلور موضعيا وذلك للحد من عملية فقدان المعادن من الاسنان وبتحفيز عملية اعادة التمعدن

- انخفاض نسبة انتاج الاحماض التى تطلقها اللوائح الجرثوميه مما يفيد فى تقوية الاسنان ونقص فى تكوين الجير والبلاك مما يساعد فى المحافظه على اللثة بشكل سليم.

طرق استخدام الفلور

من المتعارف عليه عالميا هو ان يتم اضافة مادة الفلور الى مياه الشرب بنسبه متفق عليها وهى : 1 pp، أى جزء لكل مليون، مما يساعد على التقليل من نسبةالتسوس بمعدل 50%

- يتوفر الفلور على شكل حبوب أو قطرات

- يستخدم بإشراف طبيب الاسنان اما جهازيا او موضعيا

- الحليب الذى يحتوى على الفلور بنسبة تعادل ppv-2,5 يقلل من نسبة التسوس

- الملح الذى يحتوى على الفلور يساعد فى تخفيض نسبة التسوس

- يستخدم الفلور موضعيا وذلك بشكل جل او محاليل يتم تطبيقها على سطوح الاسنان بواسطة قوالب خاصه tray وبهذا يتم تقليل نسبة التسوس الى 20 - 40 %

مثال على ذلك fluoride varnish

تتوفر ايونات الفلور فى مادة تدعى duraphate حيث تساعد على تحفيز عملية اعادة التمعدن بعد اكتشاف تسوس الاسنان فى بداياته فى طبقة المينا والعاج المكشوف

- المضمضات الطبية

- تحتوى المضمضات الطبية على الصوديوم فلورايد بنسبة 0,05% للمضمضات التى تستخدم يوميا مما يساعد على تقليل نسبة التسوس من 16 - 50 %

- مراعاة عدم استخدام الاطفال للمضمضات الطبية دون سن السابعة

- معاجين الاسنان tooth paste: تحتوى غالبية معاجين الاسنان الطبيه على الفلور NAF صوديوم فلورايد

- ان استخدام معاجين الا سنان للبالغين بنسبة 1000 - 1500 pp وذلك مرتين يوميا مما يساعد على تقليل نسبة التسوس من 10 - 30 %

- ان نسبة الفلورايد فى معاجين اسنان الاطفال تتراوح ما بين 125 - 550 pp

التسمم بواسطة مادة الفلور

عند اخذ جرعه زائده من الفلور اثناء فترة نمو الاسنان

عندما تحتوى مياه الشرب على نسبه تتعدى pp1

اعراض التسمم بالفلور

- حدوث تصبغ فى الاسنان بدرجات متفاوته منها الابيض الباهت ومنها نقاط بنية او قاتمة اللون

- تبدا الاسنان بفقدان المعادن التى تحتويها

- تظهر هذه الاعراض فى الاسنان الدائمه ولا تظهر فى الاسنان اللبنية

العلاج

يقسم التسمم بالفلورايد الى ثلاثة اقسام:

1- تسمم فلور بسيط وينتهى تدريجيا مع الوقت

2- تسمم فلور متوسط من الممكن ان تحتاج الى تبييض اسنان بالليزر او فى عيادة طبيب الاسنان

3- تسمم فلور شديد تحتاج الى اجراء تلابيس للاسنان او جسور حسب تشخيص طبيب الاسنان او طبيب التركيبات السنيه

4- حالة تسمم مميتة (32—64 MG/K.BODY WEIGHT)

5—السميه الحاده MG/K.BODY WEIGHT

الجرعه المعدله اكثر منMG 0 شرب كميات كبيره من الحليب

اقل من MG0 اجراء غسيل للمعده

وفى حال التاخر عن الاسعافات الاوليه تؤخذ : CALCIUM GLUCONAT IV
