No more Time is needed for Implant Loading with OsseoPulse System

Implant stability may be affected by initial resorption of bone at the implant surface. This may dictate the timing of implant loading. Since the discovery of osseointegration, scientists and clinicians have been investigating methods to enhance osseointegration by improving implant characteristics and enhancing the environment around the dental implant to provide optimal conditions for bone integration.

Several in vivo and in vitro studies have demonstrated that the use of photobiomodulation may improve bone repair in surgical defects. Greater and faster bone formation was seen in irradiated defects and surgical wounds when compared with control groups

Biolux Research Ltd. is pleased to announce the successful completion of a new clinical study using a shorter, more efficient, photobiomodulation treatment protocol to increase implant stability after surgery.

The clinical study was recently completed at the University of Toronto (Canada) by Mr. Mahdi Ghuloom, as part of his Master of Science thesis in the Graduate Department of Dentistry. The study investigated the efficacy of a new treatment protocol for the OsseoPulse™, Biolux’s approved photobiomodulation product, on implant stability.

A total of 72 patients requiring 76 dental implants (35 female and 37 male; mean age 63.5; range 35-100 years) were recruited and consented for inclusion into this trial. Patients received dental implants by a single operator. Implants were randomly assigned to one of two groups, control (n=47), and treatment group (n=29). All patients in the study received dental implant(s.) The treatment group had to apply light emitting diode (LED) delivered by OsseoPulse device to the surgical site, while the control group did not.

The OsseoPulse device consisted of an adjustable headset which could be customized to the patient’s face and head. Stability and repeatable positioning were achieved by ensuring three point contacts on the ear and bridge of the nose. This integrated alignment system allowed the dentist to position the LED treatment array over the surgical site on the surface of the cheek, ensuring a repeatable positioning by the patient

The new treatment protocol consists of a preoperative 20-minute phototherapy session on the day of surgery, followed by daily phototherapy sessions for the next 10 days, compared to the original 21-day treatment protocol). The results of the study show an increase in implant stability starting from day zero for the treated group, compared to the typical decrease in implant stability in the first few weeks after placement, as shown in the control group. Implant stability was assessed using the Osstell device which utilizes resonancefrequency analysis (RFA) technology and expresses data as Implant Stability Quotient (ISQ) values. The increase in implant stability with the new protocol suggests a reduced risk of implant failure and the potential for earlier or immediate loading of implants

Removing infected teeth before Heart surgery is not always wise

Currently, removing abscessed or infected teeth before heart surgery is standard practice, as it decreases infection and inflammation risks during and after the procedure. The American College of Cardiology and American Heart Association classify dental extraction as a minor procedure with a less than 1 percent risk of death or non-fatal heart attack.

But although it is standard practice to remove bad teeth prior to heart surgery, there is only limited evidence that supports this practice. The new study set out to evaluate what harms may be associated with dental extraction before cardiovascular surgery.

"Our results, however, documented a higher rate of major adverse outcomes, suggesting physicians should evaluate individualized risk of anesthesia and surgery in this patient population," says study author and anesthesiologist Dr. Mark M. Smith, from the Mayo Clinic in Rochester, MN.

Cardiac surgeon Joseph A. Dearani, MD, along with anesthesiologists Mark M. Smith, MD and Kendra J. Grim, MD, and colleagues from the Mayo Clinic in Rochester, Minn., evaluated the occurrence of major adverse outcomes in 205 patients who underwent at least one dental extraction prior to planned cardiac surgery from 2003 to 2013. The median time from dental extraction to cardiac surgery was 7 days (average 35 days).

Dr. Smith and his colleagues found that 8% of patients who had teeth removed prior to heart surgery experienced adverse outcomes. These included heart attack, stroke, kidney failure and death.

Overall, 3% of patients died after dental extraction and before the heart surgery could take place.

But this study did have some limitations. Co-author and cardiac surgeon Dr. Joseph A. Dearani says:

"With the information from our study we cannot make a definitive recommendation for or against dental extraction prior to cardiac surgery. We recommend an individualized analysis of the expected benefit of dental extraction prior to surgery weighed against the risk of morbidity and mortality as observed in our study."

In an invited commentary in the same issue of The Annals, Michael Jonathan Unsworth-White, FRCS, from Derriford Hospital in Plymouth, United Kingdom, discussed the need for surgeons to take note of the study results. "'Accepted wisdom' leads surgeons to request dental reviews prior to cardiac surgery in many thousands of patients annually around the world," said Unsworth-White. "Dr. Smith's group asks us to question this philosophy. It is a significant departure from current thinking."

Writing in a linked comment, Dr. Unsworth-White draws parallels with another recent change of consensus. In patients undergoing dental work who have existing heart problems, it has previously been standard practice to prescribe prophylactic antibiotics.

Again, this was because there is a known link between dental bacteremia and endocarditis. But more recent studies have suggested that the potential side effects of these antibiotics may outweigh the benefits.

Dr. Unsworth-White explains:

"The American Heart Association and the National Institute for Health and Clinical Excellence in the UK have withdrawn support for this practice of prophylactic antibiotics because the danger from overuse of antibiotics outweighs any other potential risks. Regular tooth brushing, flossing, and even chewing gum are now recognized to dislodge as much, if not more, bacteremia than most dental procedures."

إغسلي فمك بالزيت!

بعد التطوّر الطبي الذي نشهده حالياً، هل سنعود إلى الطبيعة والعصور القديمة لاختيار غسول الفم المناسب لصحة أسناننا؟ عاد الزيت إلى الأضواء ولكن هذه المرة ليس لاستخدامه في الطبخ بل ليكون غسولاً فعّالاً للفم من شأنه القضاء على البكتيريا الضارة.

قد تجدين من الغريب جدّاً مضمضة فمك بالزيت ولكن يرى الأطبّاء أنّ هذا النوع من السوائل يستطيع سحب البكتيريا من الفم كالمغناطيس وتماماً كما يتخلّص الشريط اللاصق من الرؤوس السوداء التي تنتشر على الأنف. وبذلك يكون الزيت قد ساهم في تجنّب ألم الأسنان، التخلّص من البقع ورائحة الفم الكريهة التي غالباً ما تنتج من البكتيريا.

وبما أنّ بكتيريا الفم بإمكانها الوصول إلى مجرى الدم والتأثير سلباً على صحة الجسم بشكل عام، فالتخلّص منها قبل أن تتمكّن من القيام بذلك يعدّ فرصة جيّدة للحفاظ على سلامتنا. ولتأكيد هذه الفكرة، قام الباحثون بدراسات عديدة ووجدوا عدّة أنواع من البكتيريا التي تسبّب التهاب اللثة على جدران شرايين القلب وهي المعروفة بتسبّبها بأمراض القلب.

وفي حين أنّ عدداً من الدراسات قد دعم نظرية تأثير الزيت الإيجابي على صحة الفم والأسنان، لم يتمكّن الطب إثبات حتّى الآن أي فائدة للمضمضة بالزيت سوى التخلّص من البكتيريا. ومع ذلك، لم تذكر المراجع الطبية أي آثار جانبية لهذه العملية لذلك بإمكانك مضمضة فمك بملعقة طعام من زيت جوز الهند العضوي. وعلى الرغم من أنّ زيت السمسم، دوار الشمس، والزيتون سيقوم بحماية فمك أيضاً، إلّا أنّ زيت جوز الهند يتميّز بخصائصه المضادة للبكتيريا وقدرته على التخلّص من الفطريات.

كيف تقومين بغسل فمك بالزيت؟ قومي بمضمضة الزيت صباحاً لمدة 5 دقائق وبعدها تخلّصي منه واغسلي فمك بمحلول الماء والملح، ومن ثمّ نظّفي أسنانك بمعجون الأسنان كالمعتاد.

عندما يتمّ تشخيص الأمراض من فمك

أصبحت صحة الفم أكثر أهمية من قبل وذلك بعد أن توصّلت العديد من الدراسات إلى كشف أمراض الفرد من خلال صحة الفم التي تعكس بشكل مباشر ما يحدث في أماكن أخرى من الجسم. ويمكن لأمراض اللثة مثلاً أن تكون خير دليل لإصابتك بمرض السكري أو الأمراض المنقولة جنسياً. بعد أن سبق وقدّمنا لك في الجزء الأول من موضوعنا "شخّصي أمراضك من خلال فمك!" بعض الأمراض التي قد يكتشفها فمك، تعرّفي معنا في ما يلي على قسم من الأمراض والمشاكل الصحية الأخرى:

- بقع الأسنان: الأمر ليس جمالياً فحسب، إذ على الرغم من أنّ بقع الأسنان قد لا تكون خطيرة إلّا أنّها تشير إلى اجتياح الجراثيم لفمك ما يعني رفع احتمال إصابتك بأمراض اللثة.

- الأسنان الحساسة: يمكن للمشروبات الساخنة أو الباردة أو المواد الغذائية أن تتسبّب بألم في الأسنان ولكن إذا كنت غالباً ما تشعرين بهذا الألم فقد يكون السبب تجاويف في السنّ، إذا تركت من دون علاج قد تمتدّ إلى الأنسجة المحيطة بالعصب.

- النزيف: إذا كت تلاحظين بضع بقع من الدم عند غسل أسنانك بعد تنظيفها بالفرشاة أو الخيط فقد يكون السبب الضغط بقوة عليها. ولكن إذا كنت تجدين أنّ كمية الدم تظهر بشكل واضح ودائم فيجب عليك مراجعة طبيب الأسنان وذلك لأنّ اللثة الدامية قد تكون علامة على التهاب الفم أو التهاب اللثة.

- القروح: تعتبر قروح الفم مزعجة ومؤلمة ولكنّها غالباً ما تختفي من تلقاء نفسها بعد أيام من دون أن تخلّف أي ضرر في الفم. ولكن إذا لاحظت ظهور قروح أو بقع بيضاء داخل أو حول الفم تدوم لأكثر من ثلاثة أسابيع فيجب عليك مراجعة طبيبك وذلك لأنّها قد تشير إلى الإصابة بفيروس الورم الحليمي البشري التي قد تؤدّي إلى سرطان الفم.

انحسار اللثة: لماذا أصبحت أسناني اكثر ظهورا؟

هل نظرت فى المرآة ذات صباح لتجد أسنانك تبدو أطول ؟ هل لاحظت أن اللثة تبدو منخفضة عن الجزء المعتاد المفترض أن تغطيه بانستجتها فوق أسنانك ؟ إذا لاحظت هذه الظاهرة فلابد ان تأخذ حذرك فقد تكون هذه بدايات مشاكل انحسار اللثة ، و هذا ما سنأخذ جولة للتعرف عليه فى هذا المقال .

إن انحسار اللثة هى حالة مرضية حيث تتباعد اللثة الواقية عن مكانها الطبيعى حول السن أو الضرس مما يظهر الأسنان أو جذورها بشكل أوضح من المعتاد وهى تعد من أول علامات مرض اللثة فعندما تنحسر اللثة تظهر فجوات وجيوب بين الأسنان تسهل عملية تكاثر البكتيريا المسببة لأمراض الأسنان وإذا لم يتم علاجها بشكل سليم فسوف تعرض الأنسجة الضامة والعظام فى الفم لضرر شديد قد يؤدى لفقدان الأسنان فى النهاية.

وتعد مشكلة انحسار اللثة من المشاكل المنتشرة بشكل كبير حتى أن البعض لا يدرك أنه مصاب بها لأنها تحدث بشكل تدريجى وتظهر أهم علاماتها على شكل حساسية الأسنان أو أن تلاحظ أن السن أصبح أطول من المعتاد حتى أنك تشعر بحزّ فى نهايته .

انحسار اللثة ليس شيئاً بسيطاً يمكنك تجاهله فإذا لاحظت أنك مصاب بانحسار اللثة فمن الضرورى أن تحدد موعد مع طبيب الأسنان فالعلاج متوفر لهذه الحالة وبإمكانه إعادة اللثة لوضعها الطبيعى ومنع أى مضاعفات.

لماذا تنحسر اللثة؟

1- التهاب الانسجة الداعمة : ويحدث ذلك عن طريق أنواع من البكتيريا التى تؤدى لتجمير نسيج اللثة والأنسجة الداعمة للسن وحتى العظام حيث أن أمراض اللثة هى أهم أسباب انحسارها.

2-عوامل جينية : بعض العوامل الوراثية قد تجعلهم أكثر عرضه لانحسار اللثة بغض النظر عن مدى إهتمامك بنظافة الفم

3- تنظيف الأسنان بشكل عنيف : إذا كنت تنظف أسنانك بشكل غير صحيح أو تعتقد أن العنف فى التنظيف سيجعلها أكثر صحة فليس ذلك صحيحاً فإن هذه العوامل تؤدى لفقدان الطبقة السطحية للأسنان وتعرض اللثة للألتهاب والأنحسار .

4- اهمال النظافة : إهمالك لتنظيف الأسنان وإستخدام الخيط الطبى يجعل المواد الغذائية المتكومه بين الأسنان تتحول إلى مادة صلبة لا يمكن إزالتها إلا عن طريق أجهزة طبية معينة وإهمالها يؤدى لأنحسار اللثة وإلتهاباتها .

5- التغيرات الهرمونية : بالنسبة للإناث فإن التغيرات الهرمونية المختلفة أثناء فترات البلوغ والحمل وما إلى ذلك يجعل الأسنان أكثر حساسية وعرضه للإلتهاب وانحسارات اللثة .

6-التدخين : الذى يسبب تكون طبقات مضرة على الأسنان تتحول مع الوقت لمواد صلبة تؤدى لتباعد اللثة .

7- طحن الأسنان والجزّ عليها : مثل هذه الأفعال تؤدى لزيادة الضغط الواقع على الأسنان.

8- لأسنان الملتوية : عدم إنتظام الأسنان أو إلتواؤها يودى لعدم توزيع الضغط بشكل متساوى على الأسنان كلها مما يؤدى لزيادى الضغط على اللثة والعظام.

9- ثقب الشفاه أو اللسان : إستخدام الحلى فى ثقب اللسان أو الشفاه يؤدى للإحتكاك المستمر مع اللثة مما يؤدى لإلتهابها وإنحسارها.

كيف تتم معالجة انحسار اللثة؟

بالنسبة للنوع البسيط من إنحسار اللثة فيقوم الطبيب بإستخدام أدوات طبية معينة بتنظيف الأسنان من تراكم المواد الصلبة والجيوب فيما بينها وصقل وتنعيم جذور الأسنان لجعلها أكثر صعوبة للبكتيريا فى الغلتصاق بها كمان أستخدام المضادات الحيوية أيضاً قد يساعد على التخلص من الإلتهابات والإصابات البكتيرية فى منطقة بين الأسنان وبين السن واللثة. أما فى الحالات المقتدمة التى لا يصلح معها التنظيف العميق نتيجة حدوث تدمير شديد للعظام والأسنان فإنه قد تحتاج لإجراء جراحة لإصلاح الأجزاء المصابة.

ماهى انواع الجراحات المستخدمة؟

-عمليات ازالة الجيوب :

حيث يقوم الطبيب بإبعاد اللثة من مكانها وتنظيف وإزاله الجيوب المصابة ثم تثبيت اللثة مرة إخرى فى مكانها الطبيعى

- عملية التجديد :

كما فى العملية السابقة يتم إبعاد اللثة وتنظيف الأماكن المصابة ثم يتم تجديد العظام أو الأسنان والإنسجة المدمرة عن طريق وضع مواد بروتينية محفزة للإنسجة أو ترقيع الأنسجة المصابة ثم إعادة اللثة إلى مكانها الطبيعى.

: يوجد العديد من أنواع هذه العملية ولكن أهمها ما يسمى بترقيع النسيج الضام حيث تؤخذ قطعة من تحت الجلد أو الغشاء المخاطى للفم والنسيج المجاور له ثم إعادة الجلد السطحى لمكانه ويتم خياطة الرقعة المأخوذة فى اللثة المصابة أو فى طريقة إخرى يتم أخذ الرقعة من سقف الفم بشكل مباشر ويقرر الطبيب نوع العملية حسب المناسب لحالتك.

الوقاية من انحسار اللثة؟

الأهتمام بنظافة الأسنان وإستخدام الخيوط الطبية يومياً هو الطريق الأفضل كمان أن اإلتزامك بفحص أسنانك مرتين سنوياً على الأقل شئ مهم فى الوقاية وإذا كنت مصابا بالفعل فقد تزيد عدد زياراتك حسب الحالة . أستخدم فرشاة ناعمة الأطراف واسأل الطبيب عن أفضل طريقة لتنظيف الأسنان بشكل سليم , وإذا كانت مشكلتك فى عدم انتظام أسنانك فاستشر الطبيب عن إجراء التقويم المناسب لك.

و بشكل عام توقف عن التدخين , وأحصل على غذاء صحى ومتوازن وراقب أى تغيرات قد تطرأ على شكل أو رائحة أسنانك فتحافظ عليها بشكل جيد.
